This paper deals with the special healthcare needs of people with chronic heart failure and attempts to identify nursing care approaches that can be successfully implemented in this area. Expanding primary care is an attractive way to ensure effective and patient-centred care for the growing number of patients with chronic heart failure. Nursing counselling should play an important role in this area and support patients in managing their illness themselves as far as possible. This means entrusting patients with symptom control and coping strategies for this chronic disease and supporting them in implementing these measures. International comparative studies suggest that this structural adjustment could lead to significant cost savings in the public health system while improving the well-being and quality of life of this patient group.
