This document contains 80 blank forms for taking a reading record during a guided reading lesson in class, or in private session. At the front of the book, there is a blank form which lists all reading levels from kindergarten through 12th grade which can be used as an overview of progress. There is a column to write the date each level is achieved, as well as a space to write the number of high frequency words mastered. On each running record form, there are sections for errors, self-corrections, frustration, independent or instructional reading level, comprehension, fluency, and self-correction rate.
