Rich’s Vascular Trauma

VASCULAR TRAUMA first published 35 years ago as the worldwide reference for vascular injury. The product has offered providers of all specialties a foundation for the diagnosis and treatment of vascular injury in both civilian and military settings. VASCULAR TRAUMA originated from the combat experience of surgeons during the Vietnam War and its publishing paralleled ground-breaking advances in surgical care in the 1970s. The 2nd edition followed much later in 2004 and provided an essential update although its content was based on, and gathered during, decades of relative peace from the standpoint of worldwide military conflict.

The attacks of September 11, 2001 ushered in a new era of injury pattern and trauma care for the world. Random, single explosive events and the multitude of injuries that resulted have spared few countries. Terrorist attacks in Madrid, London, Bali, Moscow, Islamabad and other cities have resulted in a burden injury not witnessed in decades. Additionally the wars Iraq and Afghanistan now represent the most prolonged military conflict in US history resulting in nearly 50,000 wounded service personnel to date. Because of increased survival of wounds on the modern battlefield, rates of vascular injury in combat are 4-5 times those previously reported in war. In aggregate military and civilian surgeons as well as medical institutions around the world are faced with an incomparable burden of vascular injury.

The wealth of experience in vascular injury management since 2001 has resulted in an unprecedented amount of peer reviewed publications focused on blood vessel trauma. Clinical papers describing the acute management of vascular injury in the civilian and wartime setting have been published and novel outcomes and rehabilitation reports provided. Vascular injury and surgical surge requirements in the setting of single explosive terrorist attacks have been delineated. Groundbreaking information from translatable large animal studies has advanced the understanding of ischemia reperfusion injury, resuscitation strategies, hemostatic agents, damage control adjuncts and modern endovascular techniques. Evidence-based clinical practice guidelines on the topic of vascular injury have now been developed and published in a first ever Joint Theater Trauma System to guide military surgeons to optimal practice strategies.

Despite the impressive nature and pace of medical advances in the arena of vascular trauma over the past decade, the information has appeared in disparate venues and sources of publication. At this important time in surgical history there is no one resource with which to assemble and distribute effectively the contemporary gains made in vascular injury management. Similar to the late '70s when the first edition of Vascular Trauma was published, there now exists a unique opportunity to publish a new edition of this work which will establish a modern benchmark for providers and institutions worldwide. Our goal is to create a resource - edited and supported by the major society in the specialty - that presents evidence-based treatment and management for vascular injuries worldwide.
