You won't find any scientific literature that tells you how to do a successful gradual dose reduction process, or how to do a differential diagnosis with a dementia patient, or some of the other guidance found in this book. The practice suggestions given here are a synthesis of information provided in scientific studies, as well as years of clinical practice experience and observations, pieced together into a coherent, effective clinical technique that works consistently and is safe for patients. If you are skeptical, I ask that you try some of these suggestions. Maybe when you have tried everything else and don't know what else to do, try some of these suggestions. I know that if you do, you will be pleasantly surprised by how much more efficient and effective your psychotropic medication prescribing will be and how much better patient outcomes will be. We are underestimating the value of using best practices to treat elderly patients with dementia, who also have comorbid psychiatric illness. When we use differential diagnosis and treat according to the standard of care, the beneficial outcomes can be pretty amazing and come quite rapidly. There aren't a whole lot of opportunities working with elderly dementia patients to see them getting noticeably better, but this is one area where we really can have that experience for the patient and for their loved ones and for their caregivers. This book was written with providers who work in skilled nursing facilities in mind, but it will be useful for anyone who regularly prescribes psychotropic medications for the treatment of psychiatric illness in elderly patients, and for family who want to learn more about how to make sure their loved one has the best quality of life with the fewest possible, and the safest possible, medications.
