When Nathan's family arranges for him to have a pen-pal, he writes to Claire, who lives in a hospital and cannot leave because she needs a new heart. Nathan begins sending pictures and stories to Claire, and they become close friends. When Nathan visits Claire, he discovers that many children in the hospital do not have pen-pals, so he decides to help them by finding pen-pals for them. Nathan then gives Claire his beloved toy elephant, Eli, to keep her company until she can go outside and have adventures of her own. The book encourages readers to be kind, thoughtful, and considerate of others who may be facing difficult times.
