A story of the origin of the universe, life, and a car crash

I read this out-of-print book when I was 13 and have never seen it again. Though my memory of it is clouded, I recall that three stories -- the origin of matter in the universe, the beginning of life on earth, and one man's struggle in a car wreck -- were masterfully woven together and resonated magically despite their seeming incongruity.
