Before seeing a girlfriend he thought had passed away and becoming obsessed with the thought of her flesh, Axel believed his existence in a quarantine zone to be safe.

The purpose of the quarantine zones was to protect the survivors by keeping the virus at bay. Axel starts to realise that the quarantine actually does little to protect them when he encounters Riley in an alley after believing Riley passed away from an infection a year earlier when the virus first appeared. It's too close to home for the illness.
