The History and Physical Examination Workbook: A Case-based Approach

During a typical office visit, a provider has approximately fifteen minutes to interview, examine, diagnose, and treat each patient. The History and Physical Examination Workbook: A Case-based Approach, is a must-have resource for developing these skills. Providing clinical practice in the art of performing H&P's through the use of flow models, this workbook encourages students to avoid memorization and use common sense to develop a logical approach to patients' chief complaints by allowing them to partner up as patient and provider. This clinical workbook is intended accompany History and Physical Examination texts such as History and Physical Examination: A Common Sense Approach, to speed orientation in performing history and physical examinations. Chapter 1, the Art of Interviewing , is a light exploration of the thought processes and actions behind obtaining a medical history. This includes what one does prior to walking into a patient room that will lead to a more accurate patient history. Chapter 2 discusses the components of the medical history and introduces the mnemonic CODIERS SMASH FM, a memory tool which serves as an inventory of medical history, and the review of systems. Chapter 3 presents History Flows, 18 partnered cases to practice taking medical histories. Each case has a data collection sheet the provider uses to collect the medical history through the provided mnemonic, and a blank SOAP to document the encounter. The patient is provided with a case to portray and check off the progress of the provider's questioning. The study of the physical examination begins in Chapter 4 and approaches the body in a logical progression with essentially head to toe, with 14 Physical Exam Flows culminating in the Full Physical Examination. Medical history taking and performing the physical examination are brought together in Chapter 5 in Comprehensive Flows; Objective Structural Clinical Examinations (OSCE's) The logistics of order writing is detailed in Chapter 6. Completing the workbook in entirety will help prepare students of medicine for clinical rotations and patient encounters moving from observer, to provider.
