The heartrending autobiographical non-fiction work juxtaposes opposites. The quest to ascend to a `higher station in life' defines plastic surgeon, Dr John Hill's socialization. Brought up under captivity, strictly under harness and leash by an optimistic pessimist mother, small-town merchant Myra Hill, John and younger brother Julian's positivist approach to learning, grooms them to win scholarships to high ranking Baylor Medical School. In Myra's view, if equipped with classical music knowledge, their vertical escalation of the social ladder will be guaranteed. As a medical student, his conditional life choices climax 'whirlwind fashion' when he meets and falls in love with assertive socialite and jockey, Joan Robinson, another `victim' of an upbringing under oil baron father, Ashton Robinson believes in giving Joan nothing else but `the best money can buy'. Ash doesn't only hesitate to `buy' her a husband in John, but also sponsors all the `trappings', further promising smooth glide up the social and professional ladders. John's mentor, Professor Herzfield also (ill)advises the young and vulnerable medical student into marrying a rich girl like Joan so that "he doesn't have to worry about finding another scholarship". As his choices continue to be `all made', John gets trapped. At the point he meets Ann Kurth, his chosen love of his life, John has already run out of choices. It's payback-time for vindictive Ash Robinson. John insanely unleashes his bottled-up anger, murders Joan and four maybe five other people including his own `over-dependent' brother, his `inquisitive' father, his `jealous' medical partner and friend his 'competition' Jack Ramsay, and maybe even the neighbor exterminator, with ease through irretraceable lethal prescriptions. Ann joins the frayed equation at the crunch time. A well brought up, self-confident critical thinker, Ann lives on `borrowed time' as the second wife of the peak IQ, intelligent, gentlemanly yet 'schizophrenic' and diabolically possessive, Dr Hill. Ann, who reluctantly submits to being his confidante, survives an attempted murder at the hands of her husband. The volcanically temperamental John loves her to distraction and to destruction, so she's compelled to plan her `exit' intelligently and diplomatically. He has neither qualms nor iota of shame when he parrots the mantra his influential mother brought him up to live by: `there comes a time when some people are no longer meant for this world.
