With the reduction in numbers of both American and international medical graduates applying for fellowship training in nephrology, it is reasonable to assume that there will be a relative scarcity of clinical nephrologists in the future. The increased burden of renal disease, driven largely by population increases, will need to be handled by other trained medical professionals, including family practitioners, internists, nurses, advanced practice professionals, technicians and students in health care professionals, and the role of the future nephrologist will likely evolve into that of a "regional quarterback," handling the more complex cases and providing advice and guidance to both patients and clinicians.It is with this potential future in mind that I have decided to write this pocket guide targeting mainly professionals not trained in a nephrology fellowship who are dealing with the renal-disease burden in their communities.This guide is intended not to take the place of competently trained nephrologists or replace nephrology textbooks or the judgment of the treating provider but to provide clinically relevant information toward our goal of optimizing the renal care of our patients.
