Life is a journey. Regardless of our expectations of how things should go, there are always bumps and potholes in the road. As we are going along the unexpected things happen. When that happens, we often times must regroup and face the challenges head on. We have prepared this planner to help families organize and prepare for the challenging decisions we all must make someday. An Advanced Directive is a written statement or statements of a person's wishes regarding medical treatment, often including a living will with personal instructions, made to ensure those wishes are carried out should the person be unable to communicate them to family or a doctor.

When a family member can no longer make sound decisions for their care, often times family members tend to be conflicted about what is the best decision for their loved one. This can get quite intense when everyone believes they have the right answer. The truth of the matter is only the person who is in crisis knows what they desire for themselves if this ever occurs in their lives.

For an Advanced Directive and Will to be done in a most effective way, is to have these conversations while people are healthy enough to make sound decisions for their care. If they get to the point where they are unable to care for themselves, who will be assigned to care for them, take care of the finances, make medical decisions, end of life wishes, funeral instructions and so on.
