I couldn't find the notebook I wanted, so I made one. Each page is divided into four sections, ideal for the Cornell note-taking method, with a header area and the left and bottom sections unruled. The area in the upper right, however, is dotted in a Doane-style 4x4 quadrille/lined pattern. It's for sale to the public just in case this is also something other people want but can't find. Also, all the pages are blank. It's a notebook. You're supposed to put your own notes in it. Having notes in it already would kind of defeat the purpose.
