Human Factors: The gap between the procedure and the actual operation
Aviation is often associated with technology, procedures, and complex systems. Yet behind every flight lies something far more important: people making decisions.
People working under pressure, with incomplete information, limited resources, last-minute changes, and situations that rarely unfold exactly as they were envisioned in manuals and procedures.
This book was born from a reflection that accompanied me throughout many years of working in real-world operations:
Accidents rarely begin with the error identified in the final investigation report.
They begin much earlier.
They begin when pressure becomes part of everyday routine. When deviations stop attracting attention. When organizations start relying on people to silently compensate for weaknesses within the system. And when the gap between procedures and operational reality continues to grow.
Through real aviation case studies, operational experiences, and fundamental Human Factors concepts, this book explores how people make decisions in complex environments and why highly skilled professionals can become involved in events that, in hindsight, appear difficult to understand.
But this is not a book about mistakes.
It is a book about decisions.
About risk, pressure, fatigue, organizational culture, resilience, and learning.
About what truly happens before the so-called "human error."
And about one idea that runs through every page:
Operational safety does not depend solely on procedures, technology, or regulations.
Above all, it depends on people.
Because, in the end, aviation is not just a story about aircraft.
It is a profoundly human story.
