From Scratch: A First-Generation Student's Guide to Medical School

By: Epstein, Max

Book Overview: "FROM SCRATCH: A First-Generation Student's Guide to Medical School" is a transformative guide for first-generation students aspiring to become physicians. Authored by Max Epstein and published in 2025, this book challenges the traditional assumptions of medical education, asserting that medicine doesn't just need those who followed a map, but "pioneers who learned to draw their own". The central premise is that first-generation students don't just belong in medicine-medicine desperately needs what they bring.

The book highlights that while only 18% of medical students are first-generation, these communities often experience the greatest healthcare disparities. First-generation students possess crucial qualities like cultural competency, empathy, and communication skills that can bridge existing healthcare gaps.

This guide fills a vital gap in existing pre-medical resources, which often assume inherited advantages like family familiarity with professional careers, financial resources for unpaid experiences, and established networks. It provides strategic navigation for succeeding in unfamiliar systems, frameworks for authentic presentation of one's background as a strength, resource optimization strategies, and systematic approaches to professional network building. Through the stories of real first-generation students like Samantha Marazita, Maria Elena Rodriguez, and Cody Cox, the book guides readers from pre-medical uncertainty to physician leadership, demonstrating how resilience, independent learning, and authentic motivation are powerful advantages for medical careers
