Enhanced External Counter-Pulsation: The Tip of an Iceberg

The book by Dr Shakibi, summarizes the experience with EECP over ten year period at a single center. The section on ischemic heart disease shows the beneficial effect of this modality of treatment in patients with angina pectoris who are not amenable to surgery or intervention. The experience of the author with ischemic heart failure is remarkable. Many patients with heart failure, who are incapacitated, can now find hope in EECP, beside medications. This treatment is still unknown in many parts of the world for heart failure. Dr Shakibi has used EECP in several new areas of heart disease. The application of EECP in hypertrophic cardiomyopathy, and dilated cardiomyopathy, is based on the physiologic effect of EECP as a potent lusitropic therapy. The use of EECP to relieve symptoms of heart failure in univentricular hearts following Fontan operation is quite new and again very encouraging. The reduction of systemic vascular resistance and improvement of diastolic function, both are vital for this type of patients who need a compliant ventricle with reduced afterload. Combination of EECP with stem cell therapy is another new application for EECP, which awaits further research. Mobilization of bone marrow stem cells during EECP therapy has been shown at our medical center in the swine. Enhancement of this physiologic effect of EECP by simultaneous administration of patient's bone marrow progenitor cells is quite innovative, noninvasive, and free of risk and complications. The book is presented in a concise fashion with many graphs depicting the effect of EECP on several cardiac function parameters. This work is a valuable addition to the medical literature, pointing several new applications and possible new areas for research. This work is highly recommended to the interested practitioners in the field.
