From the moment Jack arrives, malign forces converge on him and an atmosphere of dread seeps into his psyche. Intern Harper's great-grandfather controls everything on the island including the hospital administration, an unethical surgeon is free to harm patients without consequences, the Mafia somehow exists on the island and is attempting to establish a rival drug network, the island's Immortal Child spirit is for some reason intent on killing or claiming Jack, violent thugs and hoodlums abound, and Jack's own instinctual pressures intensify due to his lack of intimacy with anyone.
Within weeks, Jack is witness to medical negligence: understaffed wards, junior doctors left unsupervised, corrupt specialists driven by profit, and administrators obsessed with optics rather than safety. Preventable harm is routine-an echo of Jack's father's "death by hospital." All the evidence points corruption but no real investigation ever occurs. The line between criminal networks and institutional authority has dissolved.
Jack befriends other junior doctors, especially Harper, but remains unable to find a suitable partner. Professionally, he becomes complicit in the system he condemns. Outraged but numb, he channels his experiences into detailed documentation of the malpractice around him. No one seems to care much in a system upheld by denial, greed, and fear. He must choose whether to expose Bent River's truth and risk destruction or adapt and survive within a broken world.
Emotionally desperate, Jack finds himself drawn inexorably towards Petal Trott, an exciting woman at the center of a major crime family and someone he has been warned to avoid. As pressures escalate, Jack begins to fracture. He experiences intrusive dreams and auditory hallucinations linked to the river and the Immortal Child, blurring the boundary between psychological collapse and supernatural influence. A dying patient inexplicably calls him by his father's name, fueling Jack's paranoia that his past is entwined with the island's hidden truths.
