If you ask passengers what makes flying safe, they often mention modern aircraft, strict regulations, and sophisticated technology. Few immediately think about the health of the pilots in the cockpit.

Yet moment-to-moment safety depends not only on systems and procedures, but on the physical and mental state of the people operating them, in the cockpit.

The question of pilot health is usually framed in binary terms. Either a pilot is "fit to fly" or they are not. Regulations are written as if there is a clear line between fitness and unfitness, and as if both pilots and airlines can reliably identify where that line lies. Reality is far messier. Health fluctuates. Symptoms emerge gradually. Medications have side effects. Workloads vary. Economic pressures, career concerns, and workplace culture all influence decisions that, on paper, should be simple.

Consider a stylized but entirely typical morning. A first officer wakes at 4:15 a.m. for a 6:00 a.m. report. Her throat is sore; she slept badly; she suspects she is coming down with whatever her toddler brought home from daycare. She is not incapacitated - she can stand, think, drive, talk.

But she knows she is not at her best, and she knows the day involves four sectors, a tight turnaround at a congested hub, and forecast crosswinds at the last destination. The regulation governing her situation says, in essence: do not fly if you know or suspect you are unfit. But what does "unfit" mean at 4:15 a.m. with a sore throat and no fever?

If she calls in sick, the airline must find a standby pilot, her colleagues absorb the disruption, her sick-leave record grows by one entry, and - depending on her contract - her income may take a hit. If she flies, the odds are overwhelming that nothing observable will go wrong. She flies. Almost everyone, in her position, flies.
